Circle of Support and Care Coordination Upon Re-entry

By re-engineering the siloed path and including a robust care coordination function, Wildcat has braided together critical re-entry supports, including housing, employment and healthcare. Each individual in the program is assigned a Care Coordinator tasked to work with the individual for as long as he/she is participating in order to ensure treatment success and continuity in care. In instances that require more intensive clinical services, we ensure access and referrals to a range of evidence-based and best practice interventions offered by referral partners in our Circle of Support. These programs and interventions may include Supportive Housing, Assertive Community Treatment (ACT) services, Medication Assisted Treatment (MAT), and Crisis Respite Care.

Wildcat, in partnership with Single Stop, has designed a Benefits Calculator to ensure that maximum benefits, including food assistance, health care and housing are secured by the participants. Care Coordinators are available to help participants navigate the often complex process of accessing and securing much needed resources and supports to ease the re-entry process and ensure successful community integration.
